GRC Oatley Individual Health Care Plan Policy
A written individual health care plan will be developed for each student who has complex health care support needs. Information about such needs will normally be communicated at the time of transition to Oatley Campus by middle campus teachers or by parent communication such as the Oatley Application Form. This will be acted upon by the relevant Head Teacher Welfare for that year group.
This will include the individual health care plan (pdf 228 KB) as well as suggestions for the following attachments to be included where relevant:
- an emergency care/response plan
- a statement of the agreed responsibilities of different people involved in the student's support
- a schedule for the administration of prescribed medication
- a schedule for the administration of health care procedures
- an authorisation to contact the medical practitioner
- other relevant documents.
